This atlas displays all hospital admission rates for CHD in England including coronary revascularisation (CABG/PTCA) and mortality rates by Local Authority for financial years 2002/03, 2003/04 and 2004/05.

QOF AtlasQuality and Outcomes Framework (QOF) Atlas

QOF measures practice achievement against a range of evidence-based clinical indicators and against a range of indicators covering practice organisation and management. The current maps are for 2006/07. Older data is still available in older maps.

LHO & SE single map LHO & SE dual map An atlas of selected primary diagnoses of hospital admissions for London and South East England Residents 2004/05 & 2005/06

This atlas displays the directly age-standardised rate and count for a number of primary diagnoses for London and South East residents by Local Authority . Users can toggle between a single map (area comparison) and dual map templates (correlation with deprivation - Index of multiple deprivation, 2004) from with the application.

Hospital activity by PCT AtlasAn atlas of hospital activity data by Primary Care Trust of responsibility 2002/03 - 2005/06

This atlas displays hospital episodes grouped according to the responsible primary care trust (PCT) where the patient was treated. This does not necessarily reflect where the patient lived as they may have travelled for the treatment.

All the SVG boundaries are based on digital boundaries obtained from Ordnance Survey and are subject to Crown Copyright. You are allowed to print one copy of each image for reference purposes only.
